Colonoscopy & Endoscopy
Screening Colonoscopy:If you are being referred for a screening colonoscopy, meaning you are not experiencing any abdominal pain, bleeding, change in bowel habits, or other GI symptoms, you may not need an appointment in the office to schedule the procedure. We ask that you fill out a colonoscopy questionarre which the doctor will review.
While we encourage our patients to use the patient portal, you can also download the documents here. (link under construction)
Once you completed the documents, you may return to the office in person, or by emailing to firstname.lastname@example.org
